Indication Expansion Accelerates: First Patient Dosed in Melanoma Phase 1b/II Trial of Opamtistomig

NANJING, China, Sept. 12, 2025 /PRNewswire/ — Nanjing Leads Biolabs Co., Ltd. (“Leads Biolabs” or the “Company,” Stock Code: 9887.HK) today announced the first patient has been successfully dosed in a Phase 1b/II clinical trial (NCT07099430) evaluating Opamtistomig (LBL-024, PD-L1/4-1BB bispecific antibody) as monotherapy or in combination with other agents for the ‌first-line treatment of advanced melanoma‌‌.

Opamtistomig‌, a uniquely designed PD-L1/4-1BB bispecific antibody that simultaneously blocks PD-1/L1-mediated immune suppression and enhances 4-1BB-regulated T-cell activation. This dual mechanism has the potential to convert “cold tumors” into “hot tumors”, overcoming resistance to immunotherapy and improving response rates in hard-to-treat cancers.

In two prior clinical trials, Opamtistomig demonstrated promising efficacy and a favorable safety profile in patients with highly malignant, immune-cold extrapulmonary neuroendocrine carcinoma (EP-NEC), both as monotherapy and in combination with chemotherapy. Notably, patient enrollment was completed in August 2025 for a pivotal single-arm registration trial evaluating Opamtistomig monotherapy in EP-NEC.

In addition, multiple Phase 1b/II studies are ongoing to assess Opamtistomig combined with chemotherapy for:

  • First-line small cell lung cancer (SCLC)‌
  • ‌First-and second-line non-small cell lung cancer (NSCLC)

This newly initiated Phase 1b/II multicenter trial, led by Professor Chen Yu from Fujian Cancer Hospital, with participation from multiple hospitals, aims to evaluate Opamtistomig’s efficacy and safety in advanced melanoma as monotherapy or in combination regimens.

Opamtistomig (LBL-024) is a potential first-in-class bispecific antibody simultaneously targeting PD-L1 and the co-stimulatory receptor 4-1BB. It is the first 4-1BB-targeting bispecific antibody globally to reach the single arm pivotal trial stage as a monotherapy and holds promise to become the first approved treatment specifically for extrapulmonary neuroendocrine carcinoma (EP-NEC), a malignancy with significant unmet medical need.

Notably, 4-1BB agonism can reactivate exhausted T cells and drive robust proliferation, making it particularly promising for PD-1/PD-L1-resistant or immunologically “cold” tumors. Beyond EP-NEC, Opamtistomig has been approved for clinical trials across multiple cancer types with high unmet need, including small cell lung cancer (SCLC), biliary tract cancer (BTC), ovarian cancer (OC), non-small cell lung cancer (NSCLC), esophageal squamous cell carcinoma (ESCC), hepatocellular carcinoma (HCC), gastric cancer (GC), triple-negative breast cancer (TNBC), and malignant melanoma. Encouraging clinical activity has already been observed in SCLC, BTC, OC, and other cancer types, supporting Opamtistomig’s potential as a broad-spectrum oncology therapy.

About Melanoma

Melanoma is a highly aggressive and life-threatening cutaneous malignancy, ranking as the third most common type of skin cancer. Although it represents only about 5% of all skin cancers, melanoma accounts for approximately 65% of skin cancer–related deaths.

Recent advances in immunotherapy—particularly PD-1/L1 and CTLA-4 inhibitors—have expanded treatment options and improved outcomes for many patients. However, melanoma epidemiology in China differs significantly from that of Western populations, with a predominance of acral and mucosal subtypes, which are considered “immunologically cold tumors.”

Current immunotherapies, including PD-1/L1 and CTLA-4 inhibitors, show limited efficacy in these subtypes, with objective response rates (ORR) typically below 15%. Toripalimab, the only domestically developed PD-1 inhibitor approved as first-line therapy for Chinese melanoma patients, demonstrated an ORR of 11% and a median progression-free survival (PFS) of 2.3 months in registrational studies that primarily enrolled patients with acral and mucosal melanoma.

These data highlight a critical unmet medical need for more effective therapies for melanoma patients in China, particularly for those with acral and mucosal subtypes.

In his address, Sun Dong, Secretary for Innovation, Technology and Industry of the Hong Kong Special Administrative Region Government, highlighted Hong Kong’s full commitment to advancing technological innovation, actively integrating into the national development agenda, and establishing itself as an international innovation hub through policy support, infrastructure development, and international collaboration. He underscored AI as a vital engine for high-quality economic development, expressing Hong Kong’s commitment to joining hands with global partners to drive inclusive AI development. He highlighted Hong Kong’s ambition to serve as a frontier for AI research, technology transfer, and talent, contributing to both China’s AI strategy and global AI progress.
GTI Chairman Gao Tongqing stressed that GTI is deepening global cooperation, uniting consensus, and advancing the vision of “network-intelligence integration for inclusive AI.” GTI has launched the “5G-A×AI Integration Development Project” to identify landmark AI applications and innovations. Asia cohort of Phase 3 MARIPOSA study shows RYBREVANT® (amivantamab-vmjw) plus LAZCLUZE® (lazertinib) achieved statistically significant and clinically meaningful improvement in overall survival versus osimertinib in EGFR-mutated non-small cell lung cancer

Median overall survival in Asia cohort is projected to exceed four years, demonstrating a durable benefit and surpassing osimertinib monotherapy by more than one year

First and only chemotherapy-free combination in the first line setting to show survival improvement of this magnitude in a population with the highest-prevalence of EGFR-mutated lung cancer

SINGAPORE, Sept. 12, 2025 /PRNewswire/ — Johnson & Johnson today announced positive topline results for overall survival (OS) from the Asia cohort of the Phase 3 MARIPOSA study, evaluating amivantamab plus lazertinib as a first-line therapy for patients with locally advanced or metastatic non-small cell lung cancer (NSCLC) with epidermal growth factor receptor (EGFR) exon 19 deletions (ex19del) or L858R substitution mutations. In this cohort, where EGFR mutations are more common than in other regions, the chemotherapy-free combination met the final pre-specified secondary endpoint of OS and showed a clinically meaningful and statistically significant survival benefit over osimertinib monotherapy. Median OS improvement is projected to exceed four years, surpassing osimertinib monotherapy by more than one year.

Asia has the largest population of patients with EGFR-mutated NSCLC worldwide, with an estimated prevalence of 30 to 40 percent compared to 10 to 15 percent in Europe and the United States.[1] Despite treatment advances, approximately 30 percent of patients do not reach second-line therapy, making the choice of first treatment critical.[2] Fewer than 20 percent of patients are alive five years after diagnosis.[3]

EGFR-mutated NSCLC is more common in Asia Pacific than in other regions, which makes outcomes from the first treatment especially important,” said Byoung Chul Cho, M.D., Ph.D., Professor of Division of Medical Oncology, Yonsei Cancer Center, Yonsei University College of Medicine.* “Because of rapid disease progression and health system challenges, many patients will not have the chance to receive a second-line therapy. Therefore, the first treatment determines how the disease progresses over time. The MARIPOSA Asia cohort results show that survival can be significantly extended with amivantamab plus lazertinib in patients of Asian descent, helping them live longer.”

MARIPOSA, which enrolled 1,074 patients, is a randomized, Phase 3 study evaluating amivantamab in combination with lazertinib versus osimertinib as a first-line treatment of patients with EGFR-mutated NSCLC. The primary endpoint was progression-free survival (PFS) (using RECIST v1.1 guidelines§) as assessed by blinded independent central review (BICR). Secondary endpoints included OS, objective response rate (ORR), duration of response (DOR), second progression-free survival (PFS2) and intracranial PFS.[4]

The safety profile of amivantamab plus lazertinib was consistent with the primary analysis and no new safety signals emerged with longer-term follow-up. Phase 2 amivantamab studies suggest that using prophylactic measures can help lower the risk of adverse events such as skin reactions, infusion-related reactions and venous thromboembolic events.[5],[6],[7]

Overall survival results from the Asia cohort will be presented at an upcoming congress. The combination of amivantamab and lazertinib is approved in the United States, Europe and the Asia-Pacific region in the Japan, China, Australia, Korea and Taiwan markets for first-line treatment for patients with EGFR-mutated NSCLC based on the global MARIPOSA Phase 3 study. 

About amivantamab  

Amivantamab-vmjw, a fully-human bispecific antibody targeting EGFR and MET with immune cell-directing activity, is approved in the U.S., Europe and five countries and markets in Asia-Pacific region in combination with lazertinib for the first-line treatment of adult patients with locally advanced or metastatic NSCLC with EGFR exon 19 deletions or exon 21 L858R substitution mutations.

About lazertinib

In 2018, Janssen Biotech, Inc., entered into a license and collaboration agreement with Yuhan Corporation for the development of lazertinib (marketed as LECLAZA in South Korea). Lazertinib is an oral, third-generation, brain-penetrant EGFR TKI that targets both the T790M mutation and activating EGFR mutations while sparing wild-type EGFR. An analysis of the efficacy and safety of lazertinib from the Phase 3 LASER301 study was published in The Journal of Clinical Oncology in 2023.[8]

About Non-Small Cell Lung Cancer

Worldwide, lung cancer is one of the most common cancers, with NSCLC making up 80 to 85 percent of all lung cancer cases.[9],[10] The main subtypes of NSCLC are adenocarcinoma, squamous cell carcinoma, and large cell carcinoma.[11] Among the most common driver mutations in NSCLC are alterations in EGFR, which is a receptor tyrosine kinase controlling cell growth and division.[12] EGFR mutations are present in 10 to 15 percent of Western patients with NSCLC with adenocarcinoma histology and occur in 40 to 50 percent of Asian patients.[12],[13],[14],[15],[16] EGFR ex19del or EGFR L858R mutations are the most common EGFR mutations.[17] The five-year survival rate for all people with advanced NSCLC and EGFR mutations treated with EGFR TKIs is less than 20 percent.[18],[19] EGFR exon 20 insertion mutations are the third most prevalent activating EGFR mutation.[20] Patients with EGFR exon 20 insertion mutations have a real-world five-year overall survival (OS) of eight percent in the frontline setting, which is worse than patients with EGFR ex19del or L858R mutations, who have a real-world five-year OS of 19 percent.[21] By comparison, other common cancers, such as breast and prostate cancer have a 5-year real world OS of 90 percent and 97 percent respectively.[22]
